Vital Records/Passport Applications


For complete passport and the new passport card information visit the  U.S. Department of State Bureau of Consular Affairs. Passports are required for all air travel. Passport cards are to be used for land border travel.

Passport and Passport Card Applications can be submitted through the Oakland County Clerk's Office. Hours are Monday-Friday 8:00 a.m. to 4:30 p.m. (Our busiest hours are between 11:30 a.m. and 1:30 p.m.)

Please note: According to federal guidelines and to ensure against fraud, you may not apply for a passport at the Oakland County Clerk's Office using a birth certificate issued by Oakland County. Find the nearest location to apply for a passport or card.

Requirements:

  • Each applicant must be present when submitting the application. Applicants over 16 years must have a valid Driver's License or State I.D.
  • Both parents are required to be present for minors under 16 years old unless you bring a notarized consent form signed by the absent parent.
  • Each applicant must bring a certified copy of his/her birth certificate (or original naturalization papers) to be forwarded to the Passport Office. Your documents will be returned when the passport is issued.
  • Fill out the Passport Application (used for both passport and passport card).
Fees: For Passport For Passport Card
Two2" x2" passport-quality photographs $10.00 payable to Oakland County Clerk's Office in cash, MasterCard or Visa  $10.00 payable to Oakland County Clerk's Office in cash, MasterCard or Visa 
Applicants 16 years and older  $75.00 check or money order payable to U.S. Department of State  $20.00 check or money order payable to U.S. Department of State 
Applicants under 16 years $60.00 check or money order payable to U.S. Department of State  $10.00 check or money order payable to U.S. Department of State 
Execution fee  $25.00 payable to Oakland County Clerk's Office in cash, MasterCard or Visa  $25.00 payable to Oakland County Clerk's Office in cash, MasterCard or Visa 

Once your Passport is Processed:

  • Your passport will be sent to you by U.S. priority mail.
  • Your certified birth certificate/naturalization papers will be returned with your passport.
  • Check on the status of your passport online at the U.S. Department of State Bureau of Consular Affairs.
